ENVIRONMENTAL A O �' <br /> .S SERVICES® <br /> Exhibit A-1 <br /> Scope of Work <br /> ADS Corp ("ADS") will perform sewer flow-monitoring and depth monitoring services for the City <br /> of Everett,WA (Client) in the following Phases: <br /> Phase I — Monitoring and Ongoing Maintenance <br /> 1) Advisory Maintenance. During the course of the project and as part of ADS' focus on quality <br /> processes,the ADS the field technician will also visit as required, up to twelve times per year <br /> supporting/advising the Client's field personnel for installations, removals, battery changes, <br /> software set-up and changes. <br /> 2) Types of Services to be Supported. Installation and removal of locations, troubleshooting <br /> common faults, techniques for field repair or remedy, considerations for alarm settings, <br /> measurement intervals, fast-timing settings, blockage PREDICT review, other operational <br /> settings. <br /> 3) Response. The Client will notify ADS and provide a minimum of five (5) business days for <br /> requested service. <br /> 4) Report: A monthly alarm report will be created and sent no later than the end of the second <br /> week for the previous month. <br /> Client Responsibilities: <br /> The Client will perform the following functions in connection with this project: <br /> 1) The Client is responsible for all work performed. <br /> 2) The Client is responsible for supplying all materials. <br /> a. Note that such consumable items as batteries and antenna can be purchased online <br /> at: https://store.adsenv.com/customer/account/login <br /> b. ADS recommends that the Client buy and keep three each batteries and antenna in <br /> stock to assure an uninterrupted supply. <br /> 3) If needed, client to provide ADS traffic control services for all locations as necessary. <br /> 4) Access to the site of work with sufficient area for placement of personnel and equipment, <br /> including all right-of-way and ramps, if required. This includes but is not limited to <br /> exposing manholes and clearing easements. <br /> 5) Pay all local licenses and permits fees, included in costs. <br /> 6) Assist in obtaining and complying with any special permits. <br /> 7) If sewer line is dirty and fill of debris, ensure that selected sites have been jet cleaned to <br /> minimize hydraulic deficiencies or select an alternate location. <br /> 8) Any known information concerning bypasses, overflows, base flows, critical surcharge <br /> areas, and maintenance habits. <br /> PLEASE GO TO NEXT PAGE <br /> ADS,LLC <br /> 340 The Bridge Street• Suite 204 • Huntsville,Alabama <br /> All Rights Reserved <br />
