Exhibit "A" <br /> Scope of Services, Time of Performance, Budget, <br /> Payment, Notices, and Representatives <br /> I. Scope of Service <br /> A. Activities/Project Description <br /> The Subrecipient will be responsible for administering the WayOUT program in a manner <br /> satisfactory to the City and consistent with any standards required as a condition of providing these <br /> funds. Such program will include the following activities eligible under the Community Development <br /> Block Grant Program: <br /> Program Delivery <br /> Activity: <br /> The WayOUT will serve sixty-five (65) Households by providing up to ten (10) WayOUT seminars <br /> serving low-risk offenders/at-risk youth and their parents/caregivers. Services will be delivered with <br /> the goals of reducing recidivism, reducing/preventing youth homelessness, and improving healthy <br /> and supportive relationships within families. <br /> General Administration <br /> [Description of general administrative services to be performed in support of activities noted above] <br /> Cocoon House provides adequate administrative support for the operation of the WayOUT program <br /> including leadership, human resources, and sound financial practices. Cocoon House maintains <br /> agency policies and procedures which include an affirmative action plan, a non-discrimination policy, <br /> and grievance procedure. Cocoon House maintains an accounting, financial reporting, and financial <br /> management and operations system in order to comply with Generally Accepted Accounting <br /> Principles (GAAP).The agency also retains all necessary licenses and certifications including city <br /> and state business licenses. Program staff receive regular supervision and pro-fessional <br /> development opportunities. Program activities are overseen by agency leadership and progress <br /> reports are provided to the Board of Directors. <br />
