Exhibit "A" <br /> Scope of Services, Time of Performance, Budget, <br /> Payment, Notices, and Representatives <br /> I. Scope of Service <br /> A. Activities/Project Description <br /> The Subrecipient will be responsible for administering the Mental Health Courses program in a <br /> manner satisfactory to the City and consistent with any standards required as a condition of <br /> providing these funds. Such program will include the following activities eligible under the Community <br /> Development Block Grant Program: <br /> Program Delivery <br /> Activity: <br /> The Mental Health Courses will serve 25 Person by providing mental health courses through the <br /> College of Hope program. The College of Hope course schedule is designed to support homeless <br /> families to develop new attitudes, knowledge, skills and assets, preparing them to regain and <br /> maintain permanent housing and move towards self-sufficiency. In addition to families living in <br /> shelter and transitional shelter, the College of Hope is open to all low-income adults in Snohomish <br /> County. Course are currently offered virtually due to the COVID-19 pandemic and are expected to <br /> transition to in person offerings when its is safe to do so. When that occurs, course will be offered at <br /> the Ken Schilaty Family Learning Center (5908) Evergreen Way and the Mt. Baker View Apartments <br /> (a Housing Hope facility in North Everett for low income parents attending college, located on 1 5th <br /> Street, near HM Jackson Park. Both facilities offer childcare space for child learning experiences in <br /> conjunction with the adult courses. College of Hope will offer the following course supporting adult <br /> mental health: Anger Management, Helping Your Teenager, Single Parenting, Helping Your Child <br /> Deal with Trauma, Mental Health 101, and Stress, Grief, and Transitions. We anticipate that 75% of <br /> participants in health and wellness courses demonstrate an increase in skills related to health <br /> prevention and/or managing chronic health issues <br /> General Administration <br /> [Description of general administrative services to be performed in support of activities noted above] <br /> The Social Services Director, in coordination with the Program Manager, is responsible for full <br /> implementation, fiscal oversight, all reporting requirements of the grant and the daily management of <br /> the College of Hope, including the continued development and implementation of the Program. The <br /> Program Manager is responsible for the development of partnerships, new and/or updated course <br /> offerings, the development and implementation of support services (transportation, meals, children's <br /> program, etc.) and data collection. The Administrative Assistant is responsible for clerical tasks <br /> (media materials, enrollment, data storage, data reports, filing and storage of program materials, <br /> etc.). <br />
