CLAIM #1: SK36496 <br /> Claim Number SK36496 __ <br /> Date of Injury April 25, 2017 <br /> Description of Accident Claimant was a heavy equipment operator and was <br /> driving a Vactor truck for work and it rolled over onto <br /> the passenger side of the truck. When claimant took off <br /> his seatbelt he fell down towards the passenger side <br /> and grabbed the steering wheel with both hands. He <br /> _ _ had to pull himself out of the truck. <br /> Date Claim Received by April 25, 2017 <br /> Employer <br /> Date of Claim Allowance The Department of Labor& Industries issued an <br /> allowance order on July 14, 2017 <br /> Date Allowance Order The allowance order became final 60 days after it was <br /> Became Final received, which was on or about September 14, 2017 <br /> Nature and Extent of As a result of the industrial injury, Claimant sustained <br /> Condition and Disability bilateral shoulder injuries and has not worked since the <br /> date of injury. <br /> Claimant underwent various diagnostic studies,physical <br /> therapy, medication management, work conditioning, <br /> steroid injections, right shoulder arthroscopy, <br /> 1 decompression. rotator cuff repair and biceps tenodesis <br /> on 9/5/17. left shoulder arthroscopy, decompression, <br /> rotator cuff repair and biceps tenodesis on 5/1/18 and <br /> second surgery on left shoulder arthroscopy and repair <br /> of recurrent rotator cuff tear and lysis of adhesions and <br /> capsular release on 7/17/19. <br /> The claimant has reached maximum medical <br /> improvement with regard to the industrially related <br /> conditions.
