FORM 4.04 QUESTIONNAIRE <br />Suppliers must complete this "Questionnaire" providing the information in the same order requested below. <br />Suppliers may emphasize in their narrative any areas of their proposal that they believe exceed our requirements. <br />1. Qualifications and Relevant Experience <br />A. Briefly describe your company. Include how long the company has been in business. <br />B. Describe the qualifications of your company, its business experience, and achievements. <br />C. Describe your experience producing the proposed triple combination pumpers for <br />government agencies. <br />D. Has your company already produced the apparatus that it is being proposed? How many of <br />this apparatus has been produced thus far? Provide the name and contact information for up <br />to five (5) of those customers. <br />E. Discuss any impending changes in your organization that could impact the delivery and <br />warranty of services. <br />2. Technical Capability, Approach, and Capacity <br />A. Describe the apparatus that your company is proposing. Describe how the features of the <br />apparatus your company is proposing meet or exceed the mandatory features listed in <br />Section 2. <br />B. Provide an engine, transmission and gear ratio performance report that details the <br />performance for different engine and transmission options such as a scan report. Include <br />horsepower, acceleration, and torque. <br />C. With the apparatus fully loaded and a full tank of water, what is the angle of approach and <br />departure? <br />D. What is the turning radius? <br />E. Describe any optional cab storage for the proposed apparatus. <br />F. Provide a production timeline for this apparatus. Include dates and deliverables. <br />w <br />G. What is the wheelbase of the proposed vehicles? <br />H. What is the height for each hose bed as well as the length for the rear hose bed? <br />I. Describe the non -reflective cab window glass options from Desired Apparatus Features #6 <br />that provides the best visibility from inside of the cab through the side windows. Current <br />experience is that the side windows have a reflective quality reducing and preventing visibility <br />through the windows in certain light conditions. <br />J.
