Dear Mr. Paulen: The City's contrador, Steve's Plumbing, inspected your plumbing and drainage on September 16, 2013, to determi�e if a back�vater valve is appropriate for your property. Their recommendation is to install a backwater valve. Please review the enclosed report. Based on the inspedion report, we are proceeding ��ith the backwater valve installation process, At this time we need you to sian and return the attached Backwater Valve Authorization Fom� authorizing the Citv to install the backwater valve. VJe cannot complete the work untii the Backwater Valve Authorization Form is signed. Archer Construction, will be contading you in late November to schedule the backwater valve instaliation work. The City will provide you on-site training on the operation and maintenance of the backwater vaive. hlaintenance of the valve and thc plumbing, sewer, and drainage on your property are your responsibility. The inspection identified additional site dreinage and plumbing deficiencies in your priva.e drainage and plumbing system. These indude: . Private sid� sewer is very flat • Private side sewer is holding debris These deficiencies are your responsibilily to address, as you desire.
