<br /> it is agreed that Apollo Plumbing is not responsible for the following:
<br /> 1. Damage caused to the cuslomer's property as a result of obtaining access to a�id exposing plumbing and
<br /> drainage sysiems.
<br /> 2. Additional plumbing work beyond that specifically mentioned in lhis estimale and proposal including, bul not
<br /> limited to, that which may be required because of pre-existing plumbing code violations or additional work revealed
<br /> to be necessary as a result o( performing he speci(ied work.
<br /> 3. Any repairs, installation, removal or replacement of non-plumbing ilems or aclivities including but not limiled lo:
<br /> � concrete, paving ,asphalt, slabs, sidewalks, driveways, pat�os, pools, shrubbery, grass lawns, fences, electrical
<br /> wiring and fixtures, painting, decorations, plastering, sheetrock, and otherwall coverings, glass, carpentry, milhvork,
<br /> cabinets, floors, carpeting, (loor surfaces and preparation, roofing, flashing, sheet metal gutters, downspouts,
<br /> brick, stonework, extension walls, steel and other (ramework.
<br /> 4. Damage caused to customer's plumbing system by sewer and drain cleaning equipment when such is caused by
<br /> pre-existing de(ecls in such plumbing systems.
<br /> � Customer accepts full responsibilily for lhe prompl payment of all cosls of ihis agreemenl even though customer
<br /> . may intend to obtain reimbursement irom others such as landlords, lenants, insurance companies and tort feasors.
<br /> This proposal and said specifications shall not be allered or modified except by wrillen agreement between the
<br /> parties herelo and verbal underslandings and agreemenls �vilh represenlatives shall not be bi�ding unless set (orth
<br /> herein.
<br /> LIMITED SERVICE WARRANTY
<br /> Apollo Plumbing, ihe franchisee, warranis, to lhe extent slated herein, the plumbing repair service and drain
<br /> cleaning services furnished by it The stated period of warranty commences upon installalion or repair oi plumhing
<br /> or upon cleaning of drains.
<br /> Purchaser understands ihat Apollo Plumbing's, the franchisee, liability under lhis warranty is limited to repair,
<br /> replacement, re-cleaning or re(untl of purchaser's money and does nol exlend lo property tlamage resulting from
<br /> drains which become clogged or obstructed or (rom plumbing work which fails during the agreed upon warranty
<br /> period.
<br /> � This warranty gives you specific lega', rights. You may also have other rights 5vhich vary from state to state.
<br /> ; NOTICE TO OWNER
<br /> THE LAW REQUIRES THAT THE CONTRACTOR SHALL SUBMIT A SWORN STATEMENT OF
<br /> PERSONS FURNISHING MAT[RIALS AND LABOR 3EFORE ANY PAYMENT IS REQUIRED TO BE
<br /> MADE TO THE CONTRACTOR.
<br /> 1. Do not sign �his r,ontract until you read it or i( any spaces intended for �he agreed terms, excep� as to unavailable
<br /> in(ormation, are blank.
<br /> 2. You are enlitled to a r.opy of this conlract at Ihe time you sign it.
<br /> 3. You may al any time pay oi( ttie (ull unpaid balance due under this coniract and in doing so you may receive a
<br /> parlial rebate of the service charge.
<br /> 4. You may canc.el tl�is coniract if il is solicited in person, and you sign it, at a place other lhan the seller'S business
<br /> address, by sending notice of cancellation by certified mail retum request receipt requesled lo the seller al his
<br /> address vahich notice shall be posimarked nol Iater than midnight of the third day (excluding Sundays and holidays)
<br /> following your signing lhis contract. If you choose to cancel this contracl you must reWrn or mak? available lo the
<br /> seller at the place of delivery any merchandise, in its original condilion, received by you under this contract.
<br /> This contractor is registered to do business in ihe state in which ihis svork is periormed. (See regislration number
<br /> on the front side oi this contracl) Where required, this contractor has posled with the Stale all necessary bonds or
<br /> cash deposits for the purpose o( satisfying claims againsl lhe contraclor for negligent or improper work or breach o(
<br /> contract in the conduct of ihe contraclor's business. This bond or cash deposit may not be sufficient to cover a claim
<br /> which miyht arise (rom Ihe �vork done under your r.onlract. If any suppiier of materials used in your construclion
<br /> project or any employee of the contractor or subcontractor is not paid by the conlractor or suhcontractor on yuur
<br /> job, your propert/ may be liened lo force payment. If you wish additional protection, you may reouest the coniraclor
<br /> lo provide you �vith the original "lien release" documents from each supplier or subcontrar,for on your projecl. The
<br /> contractor is required lo provide you with furlher informatior �bout lien release documents i( you requesl it.
