EVERETT <br /> MN WASHINGTON <br /> 2022 Human Needs Contract <br /> between the City of Everett <br /> And <br /> Catholic Community Services <br /> THIS CONTRACT is made and entered into as of 1st day of January,2022,by and <br /> between the City of Everett, a municipal corporation under the laws of the State of Washington <br /> (the "City"), and Cocoon House ("Contractor"). <br /> WHEREAS, the City Council of the City of Everett has authorized the City to expend <br /> general fund monies for the project specified in this Contract in accordance with the provisions <br /> of this Contract; and <br /> WHEREAS, the Contractor desires to perform the project set forth in this Contract in <br /> accordance with the terms and conditions contained in this Contract; and <br /> WHEREAS, the City has implemented AmpliFund, an online Grant Management Software <br /> program ("AmpliFund"), for grant management, payment submission, accomplishment <br /> tracking,and additional responsibilities from the Contractor as needed for City monitoring; and <br /> NOW, THEREFORE,the City and Contractor mutually agree as follows: <br /> I. Project Scope of Services <br /> A. The Contractor shall perform or cause to be performed the project generally <br /> described as follows: Program will provide housing support services to that will support an <br /> individual to reside indoors. The Residential Counselors' primary responsibility is for daily crisis <br /> management, milieu relationship management, and effective and safe daily building operation <br /> at Clare's Place;a 65 unit, 24/7 controlled access, permanent supportive housing project for <br /> highly vulnerable, chronically homeless adults, youth and veterans. Contractor shall make <br /> every effort to engage participants in services to promote housing stability and prevent returns <br /> to homelessness. Support services shall emphasize participant choice and participation shall be <br /> entirely voluntary. Contractor shall not terminate participants from the project for any of the <br /> following reasons: (a) Failure to participate in supportive services;failure to make progress on a <br /> service plan; (b) Loss of income or failure to improve income; (c) Domestic Violence; or(d) For <br /> any other activity not covered in a lease agreement typically found in Snohomish County. <br /> This project is more specifically described in AmpliFund under Contract Number CCS- <br /> 2022. The project as described in AmpliFund is referred to in this Contract as the "Project." <br />
