TERMS AND CONDITIONS
<br /> It is agreed that Apollo Plumbing is not responsible for the follo�ving:
<br /> t. Damage caused lo the customer's property as a resuit of obtaining access to and exposing plumbin� and
<br /> drainage systems.
<br /> 2. Additional plumbing wcrk beyond that specifically mentioned in this estimale and proposal inciuding, but not
<br /> limited lo, that which may be required 6ecause of pre-existing plumb�ng code violations or addilional �vork revealed
<br /> to be necessary as a result o( performing he specified work.
<br /> 3. Any repairs, mstallation, removal or replacement of non-plumbing items or aclivities including but not limiled to:
<br /> concrele, paving ,asphalt, slabs, sidewalks, driveways, pahos, pools, shrubbery, grass lawns, fences, eLchical
<br /> wiring and fixtures, painting, decoralions, plaslering, sheelrock, and other wall covermgs, glass,carpentry, millwork,
<br /> cabinets, floors, carpeting, tloor sur(aces and preparation, roofing, tlashing, sheet metal gutters, downspouts,
<br /> brick, slonework, extension walls, steel and other framework.
<br /> 4. Damage caused to customer's plumbing system by sewer and drain cleaning Equipment when such is caused by
<br /> , pre•existing defecls i� such plumbing systems.
<br /> Customer accepls full responsibility for the prompt payment of all costs oi this agreement even though customer
<br /> may intend to oblain reimbursement from others such as landlords, ter,ants, insurance companies and tort fnasors. ',
<br /> This proposal and said specifications shall not be altered or modified excepl by writlen agreement betv,een the
<br /> parties hereto and verba� understandings and agreements with represenlativ�s shall not be binding unless set forih
<br /> ' herein.
<br /> LIMITED SERVICE WARRANTY
<br /> Apollo Plumbing, the franchisee, warrants, lo ihe exlent stated herein, the plumbing repair sen�ice and drain
<br /> cleaning services (urnished by ii The stated period of warranty commences upon inslallation or repair of plumbing
<br /> or upon cieaning of drains.
<br /> Purchaser understands thal Apollo Plumhing's, the franchisee, liabilily under this �varranty is limiled to repair,
<br /> ' replacement, re-cleaning or refund oi purchaser's money and does not extend ic properly damage resulting (rom
<br /> ' drains v�hich become clogged or obstructed or from plumbing work which faii� d�.irmg lhe agreed upon warranty
<br /> period.
<br /> This warranly gives you specific legal rights. You may also have other rights which vary from state to slate.
<br /> NOTICE TO OWNER
<br /> ! THE LAW REQUIRES THAT THE CONTRACTOR SHP,LL SJ6�41T A SWORN STATEMENT OF
<br /> PERSONS FURNISHING MATERIALS AND LABOR BEFORE ANY PAYMENT IS REQUIRED TO BE
<br /> MADE TO THE CONTRACTOR.

<br /> address, by sending nolice of cancellation 6/ ceriified mail retum request receipt reruested to the seller at his
<br /> ! address which notice shall be postmarked no� later lhan midnight of the ihird day (excluiing Sundays and holidays)
<br /> ! following your signing ihis contract. If yoi. choose to cancel this contract you must reti,rn or make available to the
<br /> � seller at the place of delivery any merchs�d+�a, in its original condition, received by yo� under ihis contracl.
<br /> � This contractor is registered to do business in the slate in �.vhich this work is performed. (Sae regis�ralion number
<br /> on the iront side oi this coNract) Where required, this centractor has posted wilh the Sfate all necessary 6onds or
<br /> cash deposils for ihe purpose of salisfying claims against the contractor for negligent or improper work or breach of
<br /> ; contract in the conduct of the contractor's business. This bond or cash deposit may nol be sufficient to cover a claim
<br /> ' which might arise (rom ihe work done under your contract. If any supplier uf materials used in your construction
<br /> projecl or any employee oi the conlraclor or subcontractor is nol paid by Ihe conlractor or subcontractor on your
<br /> job, your property may be liened to force payment. Ii you wish additional proteclion, you may request the contraclor
<br /> io provide you wilh the originai "lien release" documents from eaeh supplier or subeoNraclor on your projecL The
<br /> contraclor is required to provide you �vilh furlher informalion about lien release documents if you requesl it.
