BACKWATER VALVE AUTHORIZATION <br />f'ertnission to Insoect. The Owner grants pertnission to thc City and its conUactor to <br />inspect the plumbing and drainage of the Owner's property for the purposc of gathcring <br />information regarding whether a backwater vafve should be installed. Thc City and <br />Owner will cooperate to detcrminc a mutuslly agreeable time for the inspcction. Thc <br />inspcction is at thc City's sole cost. <br />2. Backwatcr Valve. <br />A. Citv Review of Insnection and Pertnission lo Install I3ackwa�er Valve. The City <br />will review the results of the inspection and will dcterminc whcther a backwater <br />valve should be installed. By signing this authorira�ion, thc Owncr grants thc <br />City and ils coniroctor pertnission ta install the backwater valve if the Ciry <br />dctennincs that n backwater valve should be ins�alled. This installation is at thc <br />City's sole cost. <br />B. Other Devices. There may bc other dcvices that pmvide ns good as (or grcarer) <br />protection than a backwnter valve. By signing this authorizntion, the Owner <br />grants the City and its comrecror permission to install the oiher device if thc City <br />detertnines that it should be installed. This inslalletion is at the City's sole cos[. <br />Nowcvcr, if the Owner desircs installation of the other device, but thc City has <br />dctcrmined to only install a backweter vnlve, then the Owner may inswll the othcr <br />device, and the Cily will rcimburse the Owncr up to thc insWllation cost of the <br />backwater valve. <br />C. Futurc Maintenance. Upon installation, lhe bnckwater valve or other device is the <br />Owner's property. The Owncr is exclusively responsible ro maintain it. <br />3. Other Recornmtnded Imorovements. Afler the complMion of the inspection, ihe City <br />may providc the O�mcr with a list of othcr recommcndcd improvemcnts to thc property's <br />plumbing and dminagc systems.
