BACKWATER VALVE AUTHORIZATION
d. „.„ O 1.vi Permission to Inspect. The Owner grants permission to the City and its contractor to
inspect the plumbing and drainage of the Owner's property for the purpose of gathering
information regarding whether a backwater valve should be installed. The City and
Owner will cooperate to determine a mutually agreeable time for the inspection. The
inspection is at the City's sole cost.
2. Backwater Valve.
A. City Review of Inspection and Permission to Install Backwater Valve. The City
will review the results of the inspection and will determine whether a backwater
valve should be installed. By signing this authorization, the Owner grants the
City and its contractor permission to install the backwater valve if the City
determines that a backwater valve should be installed. This installation is at the
City's sole cost.
B. Other Devices. There may be other devices that provide as good as (or greater)
protection than a backwater valve. By signing this authorization, the Owner
grants the City and its contractor permission to install the other device if the City
determines that it should be installed. This installation is at the City's sole cost.
However, if the Owner desires installation of the other device, but the City has
determined to only install a backwater valve, then the Owner may install the other
device, and the City will reimburse the Owner up to the installation cost of the
backwater valve.
C. Future Maintenance. Upon installation, the backwater valve or other device is the
Owner's property. The Owner is exclusively responsible to maintain it.
3. Other Recommended Improvements. After the completion of the inspection, the City
may provide the Owner with a list of other recommended improvements to the property's
plumbing and drainage systems. The Owner is responsible for determining whether to
implement the recommendations on the list and for paying for the implementation.
OW R:
Signature: Signature:
.//1/LE% LS),e EN6oi✓ lif/ Ff}ELk5OREit15oN
Printed Nam: Printed Name:
4ifei. 1 AO/WM-lib v`" *ke
Printed Address:
