PUBLIC WORKS <br /> May 2, 2014 <br /> Michael Okelley <br /> 630 Maulsbury Lane <br /> Everett,WA 98201 <br /> Re: Backwater Valve Installation at 630 Maulsbury Lane <br /> Dear Mr. Okelley: <br /> As you are aware, the City's contractor, Steve's Plumbing, inspected your plumbing and drainage <br /> facilities to determine if a backwater valve is appropriate for your property.Their findings indicate that <br /> installing a backwater valve on your property would be beneficial. Please review the enclosed report. <br /> At this time,we need you to skin and return the enclosed Backwater Valve Authorization <br /> Form authorizing the City to install a backwater valve at your property. Once we receive <br /> this signed form,we will proceed with the installation. The City's contractor,Combined <br /> Construction,will be contacting you in May to schedule the work.This will require entry <br /> into the building at the above address. <br /> The City will provide you on-site training on the operation and maintenance of the backwater valve. <br /> Maintenance of the valve and the plumbing,sewer, and drainage on your property are your <br /> responsibility.The inspection identified additional site drainage and plumbing deficiencies in your <br /> private drainage and plumbing system. These include: <br /> • The dirt in the yard clogs the catch basins. <br /> These deficiencies are your responsibility to address, as you desire.
