PUBLIC WORKS May 2, 2014 Michael Okelley 630 Maulsbury Lane Everett,WA 98201 Re: Backwater Valve Installation at 630 Maulsbury Lane Dear Mr. Okelley: As you are aware, the City's contractor, Steve's Plumbing, inspected your plumbing and drainage facilities to determine if a backwater valve is appropriate for your property.Their findings indicate that installing a backwater valve on your property would be beneficial. Please review the enclosed report. At this time,we need you to skin and return the enclosed Backwater Valve Authorization Form authorizing the City to install a backwater valve at your property. Once we receive this signed form,we will proceed with the installation. The City's contractor,Combined Construction,will be contacting you in May to schedule the work.This will require entry into the building at the above address. The City will provide you on-site training on the operation and maintenance of the backwater valve. Maintenance of the valve and the plumbing,sewer, and drainage on your property are your responsibility.The inspection identified additional site drainage and plumbing deficiencies in your private drainage and plumbing system. These include: • The dirt in the yard clogs the catch basins. These deficiencies are your responsibility to address, as you desire. The City is not responsible for future sewer or storm damages that may result from improper maintenance of, or deficiencies within, your private plumbing and drainage system.
