TERMS AND CONDITIONS
<br /> It is agreed thal Apollo Plumbing is not responsible for the following:
<br /> 1. Damage caused to the cuslomer's property as a result of obtaining access to and exposing plumbing and
<br /> drainage systems.
<br /> 2. Additional plum6ing work beyond that specifically mentioned in ihis estimale and proposal including, but not
<br /> limited to, that�vhich may be required because of pre-exisling plumbing code violalions or additional work revealed
<br /> � to be necessary as a result of performing he specified �vork.
<br /> 3. Any repairs, inslaliation, removal or replacement of non-plumbing ilems or activities including but not limi�ed to:
<br /> concrete, paving ,asphalt, slabs, sidewalks, driveways, patios, pools, shrubbery, grass lawns, fences, electrical
<br /> � �viring and fixlures, painting, decorations, plastering, sheetrock, and other wall coverings, glass, carpentry, millwork,
<br /> cabinets, iloors, carpeting, floor surfaces and preparalion, roofing, flashing, sheet metal gutlers, downspouts,
<br /> brick, stonework, extension walls, steel and olher framework.
<br /> ' 4. Damage caused to customer's plumbing system by sewer and drein cleaning equipment e+hen such is caused by
<br /> pre-existiog defects in such plumbing systems.
<br /> Customer accepls tull responsihility for the prompt payment of all cosls of this agreement even lhough eustomer
<br /> � may intend to obtain reimbursement irom olhers such as landlords, tenants, insurance companies and lort feasors.
<br /> This proposal and said specifications shall not be altered or modified except by �vrilten agreemenl behveen the
<br /> parties herelo and verbal understandings and agreements wilh representatives shall not be binding unless set forih
<br /> herein.
<br /> LIMITED SERVICE WARRANTY
<br /> Apollo Plumbing, the (ranchisee, warrants, to the extent stated herein, the plumbing repair service and drain
<br /> � cleaning services (urnished by it The staled period of warranty commences upon inslallation or repair of plumbing
<br /> or upon cleaning of drains.
<br /> Purchaser underslands that Apoilo Plumbing's, the iranchisee, liabilily under this warranty is limited to repair,
<br /> replacement, re-cleaning or refund of purchaser's menay and does nol extend to property damage resulling from
<br /> drains �vhich become clogged or obslructed or from plumbing work which tails during fhe agreed upon warranl}•
<br /> j period.
<br /> � This warranty gives you specific legal rights. You may also have other rights which vary from state lo state.
<br /> NOTICE TO GWNER
<br /> THE LAW REQUIRES THAT THE CONTRACTOR SHALL SUBMIT A SWORN STATEMENT OF
<br /> PERSONS FURNISHING MATERIALS AND LABOR BEFORE ANY PAYMENT IS REQUIRED TO BE
<br /> Iv1ADE TO THE CONTRACTOR.

<br /> 2. You are entilled to a copy oi this conlract at the time you sign it.
<br /> 3. You may at any fime pay ofi the (ull unpaid balance due under lhis coniracl and in doing so you may receive a
<br /> oartial rebate oi ihe service charge.
<br /> �i. You may cance! this contract if il is solicited in person, and you sign it, at a place other than the seller's business
<br /> address, by sending notice of cancellation by cerfi(ied mail return request receipt requested to the seller at his
<br /> address which notice shall be postmarked nol later than midnighl of the Ihird day (excluding Sundays and holidays)
<br /> follo�ving your signing ihis contracl. If you choose lo cancel this contract you must return or make available b the
<br /> seller at Ihe place o( delivery any merchandise, in its original condition, received by you under this contract.
<br /> This cnnlraclor is registered �o do business in Ihe slate in �vhich this vrork is per(ormed. (See registration number
<br /> on ihe front side ot this cont.�ct) Where required, thi: conlraotor has posted with the State all necessary bonds or
<br /> � cash deposits for the purpose oi salisfying claims againsl the coniractor for negligent or improper work or breach of
<br /> contract in the conduct of the contractor's business. This bond or cash deposil may not be sufficient ro cover a claim ,
<br /> which mighl arise from the work done under your contract. If any supplier of materials used in your constructior.
<br /> projecl or any employee of Ihe coNractor or subcontractor is not paid by lhe contractor or subcoMractor on your I
<br /> job, your property may be liened to force payment. If you �vish additional protection, you may request the contractor
<br /> to provide you �vith fhe original '9ien release" documents Irom each supplier or subcontractor on your projecL The
<br /> contractor is required io prcvide you with (urther information about lien release documents if you request it.
