The bidder shall complete spaces on the proposal 13 form that call for, but are not limited to, unit prices; extensions; summations; the total bid 14 amount; signatures; date; and, where applicable, retail sales taxes and acknowledgment 15 of addenda; the bidder's name, address, telephone number, and signature; the bidder's 16 UDBE/DBE/M/WBE commitment, if applicable; a State of Washington Contractor's 17 Registration Number; and a Business License Number, if applicable. Bids shall be I 18 completed by typing or shall be printed in ink by hand, preferably in black ink.
