CITY OF EVERETT SPECIAL PROVISIONS <br /> 1-02.4(1)B Prevailing Wages <br /> (******) <br /> Bidder is directed to 1-07.9(1) of these Special Provisions for requirements <br /> regarding applying payment of prevailing wage rates for employment of labor within <br /> Snohomish County. <br /> 1-02.4(2) Subsurface Information <br /> Delete the first paragraph and substitute the following: <br /> If the City has made subsurface investigation of the site of the proposed Work, the <br /> boring log data and soil sample test data accumulated by the City will be made <br /> available for inspection by the Bidders. The boring logs and soil sample test data <br /> shall NOT be considered as part of the Contract, regardless of whether such data <br /> is supplied as an Appendix to the Special Provisions or not. In addition, the City <br /> makes no representation or warranty expressed or implied that: <br /> 1. The Bidders' interpretations from the boring logs are correct, <br /> 2. Moisture conditions and indicated water tables will not vary from those <br /> found at the time the borings were made, and <br /> 3. The ground at the location of the borings has not been physically <br /> disturbed or altered after the boring was made. <br /> Supplement 1-02.4(2) by adding the following: <br /> The boring log data and soil sample data, if any, can be found in Appendix A. <br /> 1-02.5 Proposal Form <br /> Delete this section and substitute the following: <br /> The Proposal Form identifies the project and its location and describes the work. It also <br /> lists estimated quantities, units of measurement, the items of work, and the materials to <br /> be furnished at the unit bid prices. The Bidder shall complete spaces on the proposal <br /> form that call for, and are not limited to, unit prices; extensions; summations; the total <br /> bid amount; signatures; date; and, where applicable, retail sales taxes and <br /> acknowledgment of addenda; the bidder's name, address, bidder's email address, <br /> telephone number, and signature; the bidder's UDBE/DBE/M/WBE commitment, if <br /> applicable; a State of Washington Contractor's Registration Number; and a Business <br /> License Number, if applicable. Bids shall be completed by typing or shall be printed in <br /> ink by hand, preferably in black ink. The required certifications are included as part of <br /> the Proposal Form. <br /> Bidder shall submit Bidder's Proposal on the Proposal Form provided in the Contract <br /> Documents. <br /> The City reserves the right to arrange the proposal forms with alternates and additives, <br /> if such be to the advantage of the City. The Bidder shall bid on all alternates and additives <br /> set forth in the Proposal Form unless otherwise specified. <br /> 1-02.6 Preparation of Proposal <br /> Delete "unless it approves in writing" from the second sentence of the first paragraph of 1- <br /> 02.6. <br /> Revise the fourth paragraph of 1-02.6, beginning with "The Bidder shall submit with the Bid <br /> a completed Underutilized Disadvantaged Business Enterprise (UDBE) Utilization <br /> Certification", to read as follows: <br /> Contractor agrees that the Contractor shall actively solicit the employment of minority <br /> group members.
