Project: POE Combined Sewer Main Improvements Change Order No. 3 Change Order Effective Date: July 11, 2022 EXHIBIT A-DESCRIPTION OF CHANGED WORK This Change Order No. 3 modifies two (2) existing bid items to the Contract and adds one (1) new item as described below. NEW ITEM New Item # 105—Crushed Surfacing Base Course. Add new item# 105, entitled, "Crushed Surfacing Base Course" at the agreed to unit price of$43.50/Ton, by adding 277 Tons to the Contract, resulting in a$12,049.50 increase to the Contract price. MODIFIED BID ITEMS Bid Item # 5—Removal of Structures and Obstructions. Modify Bid Item# 5 entitled, "Removal of Structures and Obstructions" at the Contract unit price of$1,700.00/Ton, by adding 1,930 Tons to the Contract, resulting in a$3,281,000.00 increase to the Contract price. Bid Item # 13—Special Industrial Waste. Modify Bid Item# 13 entitled, "Special Industrial Waste" at the Contract unit price of$67.50/Ton, by adding 3,770 Tons to the Contract, resulting in a$254,475.00 increase to the Contract price. JUSTIFICATION New Item # 105—Crushed Surfacing Base Course. This new item is added to match the pavement section for work in Federal Avenue. Bid Item #5—Removal of Structures and Obstructions. This bid item exceeded plan quantity and is being modified to account for additional quantities to this bid item. Bid Item # 13—Special Industrial Waste.
