Contract title, number, start and completion dates <br />d. Contract description and service details <br />B. Disclose any legal action that is in progress or has been taken against the proposing entity or <br />individuals in the last five years. <br />C. Disclose all business transactions and relations within the last five years that may create or be <br />perceived to create a conflict of interest.
