Home visits are not conducted for a future cost <br /> projection. <br /> v.Customer may submit a request for a life care plan or future cost projection via email, <br /> phone,fax, or electronically via CorVel's CareMC website, if applicable. <br /> SCHEDULE 11 <br /> Advocacy 24/7 Nurse Triage Services <br /> Terms and Conditions <br /> CorVel shall provide to Customer the following Services related to Nursing Coordination of Care <br /> Services: <br /> • Answer calls received 24 hours a day/7 days per week on CorVel maintained Customer triage <br /> telephone line. <br /> • Instruct the caller/injured employee on first aid and or initial injury treatment. <br /> • Facilitate assessment at a designated outpatient clinic or emergency room as needed. <br /> • Follow up with the injured employee within 24 hours of initial report to evaluate current <br /> condition. <br /> • Document all calls and communicate to appropriate parties. <br /> An incident shall encompass <br /> • The nurse receipt of the initial call, <br /> • The gathering of necessary information and distribution of said information to the <br /> appropriate parties,and <br /> • One clinical follow-up to the injured employee in the first 24 hours following the initial <br /> call. <br /> SCHEDULE 12 <br /> Medication Review Services <br /> Terms and Conditions <br /> I. DESCRIPTION OF SERVICES. <br /> (a) CorVel acknowledges there is a subset of patients in the system that do not improve <br /> despite extensive diagnostic testing and numerous treatments. CorVel knows these <br /> relatively fewer cases drive a disproportionate amount of costs. Interestingly,these <br /> "creeping catastrophic"claims often present as a simple strain or sprain injury or diffuse <br /> spinal pain. There is ample data to suggest that bio-psychosocial factors can amplify, <br /> sustain or mitigate a patient's perception of disability in these cases.
