SECTION 4 — PROPOSAL SUBMITTAL REQUIREMENTS <br />4.1 SUBMITTAL REQUIREMENTS <br />Suppliers must provide a proposal which must demonstrate an understanding of the project requirements <br />as stated throughout this Request for Proposal. <br />Proposals in response to this RFP must be submitted in the order specified below. Proposal responses <br />must include: <br />1. Form 4.01 Supplier Commitment and Information (included) <br />2. Form 4.02 Price Sheet (included) <br />3. Form 4.03 Questionnaire. Narrative responses to the questions asked. Suppliers should re -type the <br />heading, question identifier and question. Then answer the questions and provide in the same order <br />requested below. Suppliers may emphasize in their narrative any areas of their proposal that they <br />believe exceed our requirements. `X) <br />4.
