ATTACHMENT F <br /> CERTIFICATION OF COLLABORATION <br /> Applicant Agency Name: City of Everett Police Department <br /> Criminal justice applicants are required to consult and collaborate with their local domestic violence and sexual <br /> assault victim services providers in the development of their grant application to ensure that their proposed <br /> activities and/or equipment acquisitions are designed to promote the safety, confidentiality, and economic <br /> independence of victims of domestic violence,sexual assault, stalking, and dating violence. Criminal justice <br /> applicants should consult and collaborate with their Community Sexual Assault Program (CSAP) and the <br /> Department of Social and Health Services (DSHS)Shelter-Funded Domestic Violence Agency. Tribal criminal <br /> justice agencies may consult and collaborate with their tribal victim services program. <br /> In satisfaction of this requirement, criminal justice agencies must provide a brief description of the consultation <br /> with, and the collaborative relationship established, between the applicant and the appropriate victim services <br /> organization/s. Applicants must also have this form signed by the authorized official of the CSAP and DSHS <br /> shelter-funded agency. In counties where these organizations are separate,the applicant may print two copies <br /> of this form to enable both authorized officials to sign.Tribal criminal justice agencies may have it signed by <br /> their tribal victim services program. <br /> Description of consultation and collaboration with community victim service agencies: <br /> In collaboration with law enforcement, domestic violence and sexual assault victim services, efforts to be <br /> made during the upcoming year include; additional LE training geared toward victims of assault, expansion <br /> of DVS services to LE for community advocacy training and the identification for additional resources to <br /> provide survivors. <br /> I certify that the above is an accurate description of the consultation with and collaborative relationship <br /> established between my agency and the applicant identified above.The proposed criminal justice activities <br /> and/or property acquisitions promote the safety,confidentiality,and economic independence of victims of <br /> domestic violence, sexual assault, stalking, and dating violence. <br /> Community Sexual Assault Program Name: <br /> Signature of CSAP Authorized Official: <br /> DSHS Shelter-Funded Domestic Violence Agency Name: <br /> Signature of DSHS Shelter Authorized Official: <br /> Tribal Victim Services Program Name: <br /> Signature of Tribal Victim Services Program Authorized Official: <br />
