Exhibit B <br /> Operational Restrictions <br /> Additional Requirements for Road Use Permit No. 55-104170 <br /> 1. The City shall be notified at least 14 days prior to log haul. <br /> 2. The DNR shall designate one CB channel to be used by all vehicle traffic for the duration of this <br /> Permit and specify this CB channel to the City at least 14 days prior to log haul. <br /> 3. Signage indicating active log truck traffic shall be placed near the Monroe Camp Road gate <br /> during log haul. <br /> 4. The Monroe Camp Road gate may not be left open or unlocked for longer than a one-hour <br /> period between trucks and shall be locked every day following haul. <br /> 5. As the City will be using this same route in 2023 for the Chaplain Gap Timber Sale,the City <br /> reserves the right to shut down DNR haul in 2023 if the increased traffic creates unacceptable <br /> safety risks. <br /> 6. All BMP's,including silt fencing,sediment traps,and/or other sediment control measures,shall <br /> be in place at all times during log haul within the watershed or on City ownership. <br /> 7. Other than in the case of an emergency,the stopping(parking/staging/idling)of vehicles is not <br /> permitted within the watershed or on City ownership. <br /> 8. Prior to log haul,the DNR shall replace the failing 36"x30'culvert on the CH-ML along the haul <br /> route within the Lake Chaplain Watershed in Section 30 of Township 29 North Range 8 East, <br /> near the junction of CH-ML and CH-11,with a new pipe of the same size or larger. <br /> 9. All vehicles must carry an Emergency Oil Spill Kit as described in the Emergency Response <br /> Section of the attached Water Quality Protection Specifications. <br /> 10. All workers must carry a map of the haul route that shows the location of the Lake Chaplain <br /> watershed.The attached Water Quality Protection Specifications shall apply when with the Lake <br /> Chaplain Watershed: <br /> Page 8 of 13 Road Use Permit No.55-104170 <br />
