Disadvantaged/Women's Business Enterprise Forms: To be filled in and signed by the <br /> bidder. <br /> a. DBE Utilization Certification Form (DOT 272-056) <br /> b. DBE Written Confirmation Form (DOT 422-031) <br /> c. DBE Trucking Credit Form (DOT 272-058) (if applicable) <br /> d. DBE Bid Item Breakdown (DOT Form 272-054) <br /> 5. Non-Collusion Declaration: To be submitted with the bid. <br /> I 6. Bid Bond: This form is to be executed by the Bidder and the surety company unless bid is <br /> accompanied by a certified check. The amount of this bond shall be not less than five percent <br /> (5%) of the total amount bid and may be shown in dollars or on a percentage basis. Cash will <br /> not be accepted. <br /> 7.
