It will 12 also list estimated quantities, units of measurement, the items of work, and the materials 13 to be furnished at the unit bid prices. The bidder shall complete spaces on the proposal 14 form that call for, but are not limited to, unit prices; extensions; summations; the total bid ' 15 amount; signatures; date; and, where applicable, retail sales taxes and acknowledgment 16 of addenda; the bidder's name, address, telephone number, and signature; the bidder's 17 UDBE/DBE/M/WBE commitment, if applicable; a State of Washington Contractor's I 18 Registration Number; and a Business License Number, if applicable. Bids shall be 19 completed by typing or shall be printed in ink by hand, preferably in black ink. The 20 required certifications are included as part of the Proposal Form. For each and 39 every DBE firm listed on the Bidder's completed Disadvantaged Business Enterprise 40 Utilization Certification, the Bidder shall submit written confirmation from that DBE firm I41 that the DBE is in agreement with the DBE participation commitment that the Bidder has 42 made in the Bidder's completed Disadvantaged Business Enterprise Utilization 43 Certification.
