EXHIBIT A <br />SCOPE OF WORK <br />Exhibit A to Professional Services Agreement <br />(Form Approved by City Attorney’s Office January 7, 2010, updated December 2, 2022) <br />Work to Be Performed <br /> <br />Each City of Everett CHIP housing rehabilitation project, in which the structure was built prior to 1978, must be <br />tested for the presence and condition of Lead Based Paint on every interior and exterior surface. The Service <br />Provider may perform Lead Based Paint risk assessments and clearance inspections, and/or a combinlation of work <br />listed below. <br /> <br />Work includes: <br /> <br />• NVL Laboratories, Inc., providing an EPA Certified Inspector/Risk Assessor to perform all work. <br /> <br />• Site testing with the XRF Spectrum Analyzer and two copies of Lead Based Paint Survey test results <br /> <br />• If Lead Based Paint is present, a definitive scope of work and options are provided for CHIP Housing <br />Inspector/Advisor and the contractor’s use <br /> <br />• Inspection of the project during the course of the work and at the end, performing all required clearance <br />sampling and providing one bound copy and one electronic copy of the report <br /> <br />• Performing wipe and soil sampling, including delivery costs and lab analysis <br /> <br />• Prepare scope of work for Lead Based Paint bidding purposes <br /> <br />• Solicit contractors for bidding and conduct project walk-through <br /> <br />• Receive contractor bids and prepare analysis for Owner of project <br /> <br />• On site observation of contractor to ensure compliance of regulations <br /> <br />• Teach HUD approved lead RRP safety training program (renovation, repair, & painting). <br /> <br />• On site air monitoring, if applicable <br />
