AQUAUS <br />Services Completed For: WinCo <br />Facility #: 104 Everett <br />9900 191h Ave SE <br />Everett, WA 98208 <br />Approved Scope of Work <br />AQUALIS shall provide the Services and Deliverable(s) as follows: <br />• Mobilize all equipment and labor to the site <br />• Install necessary BM Ps, barricades, and signage as needed <br />• Jet/vac to clean and clear pipe and remove all water and material from basin <br />• Perform CSE to install 12" Tide flex valve <br />• Perform general site clean-up <br />• Demobilize & dispose of all accumulated waste at an appropriate offsite facility <br />Special Project Notes <br />• 'This proposal is valid for 30 days <br />RO_10451 <br />Project Synopsis and Completion Notes <br />AQUALIS dispatched to the site and performed a (et/vac to clean the catch basin. Once cleaned, AQUALIS used CSE to <br />enter the unit and install the 12'tide flex valve.
