<br />Page 16 of 24 <br />FORM 4.0 3 QUESTIONNAIRE <br />Suppliers must complete this “Questionnaire” providing the information in the same order requested <br />below. Suppliers may emphasize in their narrative any areas of their proposal that they believe exceed <br />our requirements. <br />1. Qualifications and Relevant Experience <br />A. Briefly describe your company. Include how long the company been in business. <br />B. Describe the qualifications of your company, its business experience and achievements. <br />C. Who are you proposing to be the project manager if awarded this contract? What is their <br />experience with this work and other aspects pertinent to this project? What is their years of <br />experience, years in industry, years with firm, year of applicable licenses, and etc? Provide a <br />list of three similar projects that person has been involved in and their role. <br />D. Provide names, tenure, roles and responsibilities for each key team member engaged in <br />providing the services. <br />E. What is your staff turnover rate annually for the past three years? <br />F. What characteristics most distinguish your organization from your competitors? <br />2. Technical Capability, Approach, and Capacity <br />A. What is your approach to this project? List the primary features or work tasks. Describe your <br />execution, management, and control of the project. <br />B. Provide a timeline plan for this project and include any City of Everett staff time <br />requirements. <br />C. How will we benefit from your approach? <br />D. Describe how your services will help us achieve and maintain our goals as listed in Section 2. <br />E. What is your availability for this project? Please include a statement of other work currently <br />under way or anticipated to be in progress during the time frame of this project and show <br />how you intend to schedule projects so this project is accomplished as well. <br />F. Does your firm intend to subcontract any portion of this contract? If so, please provide the <br />following: name of the firm(s), the percentage of work to be performed by each <br />subcontractor, and a description of the nature of work performed by each. <br />G. Describe the firm’s capabilities in relation to the fiscal requirements of this proposal. <br />H. Describe your process to ensure compliance with Washington State and Federal Transit <br />Administration laws and regulations. <br />3. Communication, Customer Services, and Training <br />A. Describe how your company project manager will keep City of Everett timely informed of any <br />issues related to delivering the services described in this RFP.
