EXIBIT A <br />Scope of Service <br />The purpose of this contract is to provide an AHERA survey on floors 1, 2, 3, 4, 5 (Humane Resources <br />Storage Room 506 and Conference Room 508), 7, 8, and building roof for the Everett Municipal <br />Building-Public Works Tenant Improvements Project located at 2930 Wetmore Ave., Everett WA 98201. <br />The survey is for the upcoming renovations. Hawk Environmental Services, Inc. will provide neutral, <br />unbiased, investigative, and testing services related to hazardous materials and environmental health <br />and safety. <br />Hawk Environmental Services provides the testing and inspection services necessary to meet state and <br />local codes or to qualify for rebates and will also design site-specific testing procedures to identify or <br />quantify any indoor environmental concern. <br />Scope of Testing: <br />Asbestos (ADHERA Good Faith Survey): <br />Visual assessment of all accessible areas of building materials; with respect to renovations on multiple <br />floors and the roof. <br />•Create a list of all potentially asbestos containing building materials. <br />•Sampling of suspect materials in accordance with state, local, and federal <br />guidelines (AHERA building inspector). <br />•Lab analysis (PLM) of each sample (up to 3 layers per sample) with a 10 <br />business day turnaround-time from last day of sampling. <br />•Estimated number of samples: 241-325 <br />•Apply patches: <br />•Final report will include: <br />◦Narrative <br />◦Floor-plan with sample locations <br />◦Digital photos <br />◦List of all potentially asbestos containing materials to be disturbed during <br />the work <br />◦Final lab data and pertinent interpretations <br />Assumptions: <br />To prevent conflict-of-interest, Hawk Environmental, Inc. does not provide cleanup, remediation, <br />abatement, or installation services. <br />Exhibit A to Professional Services Agreement <br />(Form Approved by City Attorney’s Office January 7, 2010, updated December 2, 2022)
