<br /> <br /> 1 <br /> <br />2023 Human Needs Contract <br /> <br />between the City of Everett <br /> <br />And <br /> <br />Catholic Community Services <br /> <br /> THIS CONTRACT is made and entered into as of 1st day of January, 2023, by and between the <br />City of Everett, a municipal corporation under the laws of the State of Washington (the "City"), and <br />Catholic Community Services ("Contractor"). <br /> <br /> WHEREAS, the City Council of the City of Everett has authorized the City to expend general fund <br />monies for the project specified in this Contract in accordance with the provisions of this Contract; and <br /> <br /> WHEREAS, the Contractor desires to perform the project set forth in this Contract in accordance <br />with the terms and conditions contained in this Contract; and <br /> <br /> WHEREAS, the City has implemented AmpliFund, an online Grant Management Software <br />program (“AmpliFund”), for grant management, payment submission, accomplishment tracking, and <br />additional responsibilities from the Contractor as needed for City monitoring; and <br /> <br /> NOW, THEREFORE, the City and Contractor mutually agree as follows: <br /> <br />I. Project Scope of Services <br /> <br /> A. The Contractor shall perform or cause to be performed the project generally described as <br />follows: Program will provide housing support services to that will support an individual to reside <br />indoors. The Residential Counselors’ primary responsibility is for daily crisis management, milieu <br />relationship management, and effective and safe daily building operation at Clare’s Place; a 65 unit, <br />24/7 controlled access, permanent supportive housing project for highly vulnerable, chronically <br />homeless adults, youth and veterans. Contractor shall make every effort to engage participants in <br />services to promote housing stability and prevent returns to homelessness. Support services shall <br />emphasize participant choice and participation shall be entirely voluntary. Contractor shall not <br />terminate participants from the project for any of the following reasons: (a) Failure to participate in <br />supportive services; failure to make progress on a service plan ; (b) Loss of income or failure to improve <br />income; (c) Domestic Violence; or (d) For any other activity not covered in a lease agreement typically <br />found in Snohomish County. <br /> <br />This project is more specifically described in AmpliFund under Contract Number CCS - 2023. <br />The project as described in AmpliFund is referred to in this Contract as the “Project.” <br />
