OTHER – PLAN CHANGE PROPOSALS: <br /> <br />Home Health Care <br />Initial three days of home health care services are covered under the HMA medical plan. <br />Services needed beyond three days are subject to Board approval upon review of <br />written physician statement and written quote from home health care provider. <br /> <br />Durable Medical Equipment <br />Durable medical equipment and supplies include items such as crutches, diabetic <br />supplies, wheelchairs, walkers, scooters, hospital beds, oxygen equipment, CPAP <br />devices, etc. A written prescription and pre-approval are required by Medicare and/or <br />the Pension Board. Retirees on Medicare must use a "Medicare contracted supplier" <br />and follow Medicare requirements when renting or purchasing durable medical <br />equipment. A list of Medicare contracted suppliers is available on their website at <br />www.Medicare.gov/supplier or by contacting Medicare at 1-800-633-4227. <br />If you are unable to purchase or rent durable medical equipment from a Medicare <br />contracted supplier or the durable medical equipment is not covered by Medicare, you <br />must submit a request for pre-approval from the Pension Board if the cost exceeds <br />$2,000. All requests must include a written prescription from a physician, written quote <br />from the supplier indicating cost of the equipment, and a written statement from the <br />member regarding why they are unable to purchase or rent from a Medicare contracted <br />supplier. Benefit levels will be determined on a case-by-case basis. <br />
