2 <br />BASIC PROVISIONS <br />Maximum Compensation Amount $40,000 <br />Eligible Expenses (not-to-exceed) $4,000 <br />Method of Payment (must select one box) <br />☐ Lump Sum paid upon completion of all work. <br /> <br />☐ Payment method is described in scope of work. <br />☒ Payment method is as follows: $6,000 per month, for 6 months, billed <br />monthly in arrears which includes all expenses (except for basic travel costs for <br />a visit to Everett, with prior written approval). Payment shall be made to Keller <br />Partners on or before the fifteenth (15th) of each month at the address listed <br />on the invoice(s). Federal funds may not be used to pay Keller Partners’ <br />professional fees. <br /> <br />State Retirement Systems (must answer both questions) <br />Does Service Provider have 25 or more employees? <br /> <br />Answer: No <br /> <br />If Service Provider has less than 25 employees, did any Service Provider <br />Personnel who will work under this Professional Services Agreement retire <br />under a DRS retirement system? <br /> <br />Answer No <br /> <br />“DRS retirement system” refers to any of the following Public Employers’ <br />Retirement System (PERS), School Employees’ Retirement System (SERS), <br />Teachers’ Retirement System (TRS), and Law Enforcement Officers and <br />Fire Fighters plan (LEOFF). <br /> <br />“Service Provider Personnel” includes Service Provider employees and <br />owners (such as shareholders, partners or members). If the Service <br />Provider is a sole proprietor, then “Service Provider Personnel” refers to <br />the sole proprietor. <br /> <br />END OF BASIC PROVISIONS <br />
