<br />Exhibit "A" <br />Scope of Services, Time of Performance, Budget, <br />Payment, Notices, and Representatives <br /> <br />I. Scope of Service <br /> <br />A. Activities/Project Description <br /> <br />The Subrecipient will be responsible for administering the Mental Health & Therapeutic Services <br />program in a manner satisfactory to the City and consistent with any standards required as a <br />condition of providing these funds. Such program will include the following activities eligible under <br />the Community Development Block Grant Program: <br /> <br />Program Delivery <br /> <br />Activity: <br /> <br />The Mental Health & Therapeutic Services will serve 20 Persons by providing evidence-based <br />mental health interventions to residents of affordable housing units and to enrollees at Tomorrow’s <br />Hope in Everett. <br /> <br />General Administration <br />[Description of general administrative services to be performed in support of activities noted above] <br /> <br />The Chief Operating Officer is responsible for the fiscal oversight and daily management of the <br />program. The Mental Health Therapist, overseen by the Chief Operating Officer, is responsible for <br />program implementation, tracking participant progress, and meeting program goals and outcomes. <br />Progress is measured via various assessment scales (SUDs, anxiety/depression scales), client <br />report, or when reviewing treatment plans <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br />
