City of Everett Inductive Charging Infrastructure for Field Installation 00 21 13 - 8 <br /> 2. Bid Signature Sheet: To be filled in and signed by the Bidder. <br /> 3. Subcontractors Form: To be filled in by the Bidder. <br /> 4. Bid Security: This form is to be executed by the Bidder and the surety company <br /> unless Bid is accompanied by a certified or cashiers check. The amount of the <br /> bond or cashiers check, which shall be not less than five percent of the Total Bid, <br /> may be shown in dollars or on a percentage basis. <br /> 5. RCW 35.22.650 Certification: To be filled in and signed by the Bidder. <br /> 6. Non-Collusion Affidavit: To be signed, notarized and submitted with the Bid. <br /> B. Failure to complete and fully execute the aforementioned forms and to submit them with ' <br /> the Bid may result in rejection of Bid. <br /> C. The following forms are to be executed after the award of Contract: I 1. Contract: This Contract to be executed by the successful Bidder in duplicate within <br /> 14 calendar days after the award date. <br /> 2. Payment and Performance Bonds: These forms are to be executed by the <br /> successful Bidder and his surety company in triplicate. The amount of these bonds <br /> shall be 100 percent of the Contract Sum and shall be submitted with the Contract. <br /> 3. Proof of Insurance: Insurance certificates and endorsements shall be obtained <br /> and maintained in force in accordance with SECTION 00 72 00, Part 15 "Liability <br /> and Insurance", of the Contract Documents. <br /> 4. Power of Attorney: Attorneys-in-fact who sign bid bonds or contract bonds must <br /> file with each bond a certified and effectively dated copy of their Power of Attorney. <br /> 5. Statement of Intent to Pay Prevailing Wage(L&I Form F700-029-000)and Affidavit <br /> of Wages Paid (F700-007-000) from the Contractor, Subcontractor and agents to <br /> the Subcontractor shall be submitted to the Employment Standards Division, State <br /> Department of Labor and Industries, Olympia, Washington. If the prime contract <br /> is over$1,000,000, then Contractor also needs to submit L&I Form F700-164-000 <br /> (Affidavit of Wages Paid EHB 2805 Addendum). <br /> 6. Weekly Statement with Respect to Payment of Wages (U.S. Dept of Labor Form <br /> WH347): Contractors, Subcontractors, and agents to Subcontractors using Payroll <br /> Form WH347) may use State of Compliance found on back of form. <br /> 7. Weekly Statement of Compliance(U.S. Dept of Labor Form WH 348). Contractors, <br /> Subcontractors, or agents to Subcontractors not using Payroll Form WH347 shall <br /> attach the Statement of Compliance Form WH 348 to each payroll. <br /> 8. Approval of Subcontractors: Contractors shall request approval of Subcontractors <br /> on a form provided by the City prior to their working on the Site. <br /> 9. Construction Progress Schedule: To be submitted as required by Section 00 72 <br /> 00, Part 5 "Progress and Completion", of the Contract Documents by the <br /> Contractor within ten calendar days, or such time as determined by the City, after <br /> the date of receipt of Notice to Proceed. <br /> 10. Traffic Control Plan: Standard Traffic Control Plans are included in the City's <br /> Design and Construction Standards. If, and to the extent, such plans are not <br /> sufficient, complete or adequate to support the Bidder's planned means and <br /> methods of performing the Work, the Bidder must develop an adequate, complete <br /> and sufficient traffic control plan at its cost, that shall be submitted for the City's I <br /> approval prior to construction. Deviation from the Standard Plans must be <br /> submitted by the Contractor and approved by the City prior to construction. <br /> 11. Disposal Sites: Provide the City with the location of all disposal sites to be used, <br /> and also provide copies of the permits and approvals for such disposal sites.
