How does your company address repairs that cannot be completed on-site at the apparatus <br />location? <br />3. Communication, Customer Service, and Training <br />A. Describe your response timeline to a warranty repair call. <br />B. Explain the process for inspection prior to acceptance and access during manufacturing that is <br />required by the City of Everett. <br />C. Describe any operation and training manuals that your company will provide if awarded the <br />bid. <br />D. Describe your delivery orientation and apparatus training. Include the format of the <br />orientation and training, duration and number of staff included. If the supplier has optional <br />formats, include the cost in Part C of the Price Sheet. <br />4. Risk, Performance, and Quality Assurance <br />A. Submit no more than five (5) completed relevant contract awards, within the past five years <br />that demonstrate successful contract performance similar in size and scope as described in <br />this RFP, include any government experience. Include the following for each reference: <br />a. Company name and full address <br />b. Point of contact name, title, e-mail address, and phone number <br />c. Contract title, number, start and completion dates <br />d. Contract description & order details <br />B. Have you defaulted on any contracts within the past three years or failed to meet contract <br />terms? If so, describe. <br />C. If your firm has had a contract terminated during the past five years, list all such incidents. <br />Submit full details of all terminations experienced by the proposer during the last five years <br />including the terminating party’s name, address, and telephone number. Present the <br />proposer’s position on the matter. <br /> <br />
