Levels of Accomplishment - Goals and Performance Measures <br /> <br />In addition to the normal administrative services required as part of this Agreement, the Subrecipient <br />agrees to provide the following levels of program services: <br />Activity Units of Service* per 90 Days Total Units/Year <br />Housing and Case <br />Management 25 100 <br />Total People or Households to be served Total unduplicated Everett Residents or <br />Households to be served <br />32 128 <br />*Note: Units of Service is defined as persons assisted. <br /> <br />The Subrecipient agrees to track outcome performance measures as required by the U.S. Department <br />of Housing and Urban Development (HUD) and the City. The Subrecipient will report the outcomes to <br />the City quarterly at minimum. <br /> <br />Reporting forms must be in the format acceptable to the City of Everett. Exhibit “E” shall be used <br />for all general reporting requirements under this agreement. <br /> <br />E. Staffing <br />[List of staff and time commitments to be allocated to the activity specified in I.A. above] <br /> <br />Project Director: The Project Director is the lead staff person for the Faith Family Village project and <br />supervises the work of other program staff on the site. This position is for 20 hours per week. <br /> <br />Family Support Specialist: The Family Support Specialist will onboard new residents, create an <br />action plan with each family of steps to take to follow to meet the goal of permanent housing, <br />coordinating the care teams that will support residents, preparation, and submission of resident case <br />management reports, and carry out all corrective action practices under Interfaith supervision. This <br />position will be for 30 hours per week. <br /> <br />Operations Manager: The Operations Manager is responsible for facility operations, managing <br />vendors and staff, maintenance of the structures and property, and janitorial service. Will work 20 <br />hours a week. <br /> <br /> <br />II. Time of Performance <br /> <br />Services of the Subrecipient shall start January 24, 2023 and end June 30, 2025. The term of this <br />Agreement and the provisions herein shall be extended to cover any additional time period during <br />which the Subrecipient remains in control of EDI funds or other assets, including program income.
