CITY OF EVERETT SPECIAL PROVISIONS <br /> <br />Beverly Lake Water Quality Retrofit Division 5 – SURFACE TREATMENTS August 2023 <br /> <br />AND PAVEMENTS <br />WO No. – UP3745 SP - 178 <br />2. A copy of each intersection’s traffic control plan. <br />3. Haul routes from Supplier facilities and locations of temporary <br />parking and staging areas, including return routes. Describe <br />the complete round trip as it relates to the sequencing of <br />paving operations. <br />4. Names and locations of HMA Supplier facilities to be used. <br />5. List of all equipment to be used for paving. <br />6. List of personnel and associated job classification assigned to <br />each piece of paving equipment. <br />7. Description (geometric or narrative) of the planing and paving <br />sequence schedule and intended area of planing and of <br />paving for each day’s work, the directions of proposed planing <br />and paving, sequence of adjacent lane paving, sequence of <br />skipped lane paving, intersection planing and paving <br />scheduling and sequencing, and making of proposed timely <br />notifications and coordination. Also show HMA joints relative <br />to the final pavement marking lane lines. <br />8. Names, job titles, and contact information for field, office, and <br />plant supervisory personnel. <br />9. A copy of the approved Mix Designs. <br />10. Tonnage of HMA to be placed each day. <br />11. Approximate times and days for starting and ending daily <br />operations. <br />5-04.3(14)B3 Pre-Paving and Pre-Planing Briefing <br />At least two Working Days before the first paving operation and the <br />first planing operation, or as scheduled by the Engineer for future <br />paving and planing operations to ensure the Contractor has <br />adequately prepared for notifying and coordinating as required in the <br />Contract, the Contractor must be prepared to discuss that day’s <br />operations as they relate to other entities and to public safety and <br />convenience, including driveway and business access, garbage truck <br />operations, transit operations and working around energized <br />overhead wires, school and nursing home and hospital and other <br />accesses, other contractors who may be operating in the area, <br />pedestrian and bicycle traffic, and emergency services. The <br />Contractor, and Subcontractors that may be part of that day’s <br />operations must meet with the Engineer and discuss the proposed <br />operation as it relates to the submitted planing plan and paving plan, <br />approved traffic control plan, and public convenience and safety. <br />Such discussion includes, but is not limited to: <br />1. General for both Paving Plan and for Planing Plan: <br />a. The actual times of starting and ending daily operations. <br />b. In intersections, how to break up the intersection, and <br />address traffic control and signalization for that operation, <br />including use of peace officers. <br />c. The sequencing and scheduling of paving operations and <br />of planing operations, as applicable, as it relates to traffic <br />control, to public convenience and safety, and to other <br />con-tractors who may operate in the Project Site.
