3 <br /> <br /> Overview Map <br />The District requires that all submissions be supplied with an overview map. An example of this <br />map is shown on Figure 3. The map can be formatted however the applicant wishes, but must <br />include the following information: <br /> The application reference identification. <br /> A color-coded legend to distinguish between new connections, overlash, fiber vs. coax, and <br />existing connections. <br /> Overhead satellite map of the route zoomed in as close as possible while showing entire <br />route or polygon. <br /> Roadways labeled so the location of the route can be easily identified. <br /> Map shall be highlighted to show the proposed route for new attachments, the tie into <br />existing facilities and poles that will be skipped. <br /> Pole number sequencing must align with Pole Maps, Photos and Fielding Spreadsheet <br /> <br /> <br /> <br /> <br /> Fielding Sheet <br /> Include the application reference identification on the Fielding Sheet. <br /> The District has a Fielding Sheet (Microsoft Excel spreadsheet) in which all measurements <br />must be entered. To obtain a digital copy of the District's sheet, contact the District's Joint <br />Use department. <br /> All measurements shown on the Fielding Sheet must be submitted when present on the <br />pole. For example, if there is no streetlight present, the streetlight measurement is not <br />required. The Applicant's measurements are expected to have an accuracy of +/- 0.5". If <br />there is reason to suspect that the accuracy is not within this tolerance, the District may <br />deny the application. <br /> All measurements must be taken at the lowest point of the hardware being measured (i.e. <br />the lowest point of a streetlight bracket, not the actual height of the light). See Figure 4 for <br />an example of this sheet. <br /> Any application that includes transmission poles must include a groundline circumference <br />measurement on the fielding sheet. Note: that all applicants shall provide their own anchor <br />on transmission poles. <br /> Pole number sequencing must align with Pole Maps, Overview Map and Photos. <br /> Indicate whether attachment is fiber or coax on the sheet. <br /> If requesting Ziply to move due to them being too high, note this specifically on your <br />fielding sheet so we know why the request is being asked for. <br /> <br /> <br /> Commscope Report (not required for small cell applications) <br /> Include the application reference identification on the report. <br /> The applicant must provide a Commscope SpanMaster® cable sag and tension report for <br />all attachments. See Figure 5 for an example of this sheet.
