2 <br />BASIC PROVISIONS <br />Completion Date December 31, 2025 <br />Eligible Expenses <br />(not-to-exceed) $0 <br />Maximum <br />Compensation <br />Amount <br />$18,000 <br />Note: the Maximum Compensation <br />Amount is inclusive of Eligible <br />Expenses. <br />Method of <br />Payment <br />(must select one) <br />☐Lump Sum paid upon completion of all work. <br />☐Payment method is described in scope of work. <br />☒Payment method is as follows: Service Provider will be paid $45/hr. <br />Provider to submit monthly invoice to project manager, in the first week <br />of the month for work done the prior month, i.e. work done in May will <br />be invoiced first week of June. <br />State Retirement <br />Systems (must <br />answer both <br />questions) <br />Does Service Provider have 25 or more employees? <br />Answer: No <br />If Service Provider has less than 25 employees, did any Service Provider <br />Personnel who will work under this Professional Services Agreement retire <br />under a DRS retirement system? <br />Answer: No <br />“DRS retirement system” refers to any of the following Public Employers’ <br />Retirement System (PERS), School Employees’ Retirement System (SERS), <br />Teachers’ Retirement System (TRS), and Law Enforcement Officers and <br />Fire Fighters plan (LEOFF). <br />“Service Provider Personnel” includes Service Provider employees and <br />owners (such as shareholders, partners or members). If Service Provider is <br />a sole proprietor, then “Service Provider Personnel” refers to the sole <br />proprietor. <br />END OF BASIC PROVISIONS
