9. The ventilation system shall be installed, operated, and maintained to meet applicable building, <br /> mechanical, and fire codes. <br /> After you have passed your preoperational inspection, the annual operating permit application and all <br /> fees must be received by Snohomish Health District prior to opening for business. This facility will be <br /> classified as a high risk food establishment with 0-50 seats. <br /> Your application for a food service establishment permit from the Snohomish Health District may be <br /> approved during pre-operational inspection. However, it is the responsibility of the food service <br /> establishment operator/owner to obtain all necessary permits and approvals from all applicable <br /> agencies. Operating the establishment without required permits or approvals may subject the <br /> operator/owner to legal action by the appropriate agencies. If the establishment is opened without the <br /> Snohomish Health District preoperational inspection, it may be subject to closure. <br /> Changes or additions to the approved plans or equipment require pre-approval from the Snohomish <br /> Health District prior to implementation of the changes. <br /> A copy of the Snohomish Health District Appeals procedure is available upon request.
