8-08 FINAL UTILITY ADJUSTMENT TO FINISH GRADE <br /> A. General <br /> 1. All utility covers, valve boxes, manholes and monument cases located on asphalt <br /> roadways shall be temporarily placed at subgrade elevation prior to placing base <br /> material. <br /> 2. Final adjustment of all covers and access entries shall be completed within 15 <br /> business days following final paving, unless a different time frame is approved <br /> by the County Engineer. The covers and access entries shall be adjusted <br /> pursuant to the applicable utilities standards and specifications, provided that if <br /> there are none, the final adjustment shall be completed as follows: <br /> i. Saw-cutting or neat-line jackhammering of the pavement around lids and <br /> covers. The opening should not be larger than 12 inches beyond the <br /> radius of the cover; <br /> ii. Removing base material, surfacing course, and frame; adding raising <br /> bricks; replacing frame and cover to finish grade; <br /> iii. Pouring 5 inches of concrete around the structure and frame within no less <br /> than 2 inches from the top; <br /> iv. Filling the remaining with HMA class 1/2", compacted and sealed to provide <br /> a dense, uniform surface; and <br /> v. The maximum height differential from the finished grade and top of frame <br /> shall not exceed one-quarter inch. <br /> 3. In the event the final adjustment of all covers and access entries are not <br /> completed within the time required or approved pursuant to 8-08.A.2) above, <br /> the responsible: <br /> i. Utility shall be required to obtain a new or separate Type D-8 Major Utility <br /> Construction permit and may be required to obtain a new or separate Tvge <br /> BB-55 Traffic Control permit; or <br /> ii. Developer shall be required to obtain a new or separate Type D-5 Major <br /> Construction permit and may be required to obtain a new or separate Type <br /> B-5 Traffic Control permit. <br /> 8-09 RESTORATION REQUIREMENTS <br /> See Standard Drawing 8-040 <br /> A. Preservation, Restoration and Cleanup <br /> 1. All work shall comply with Chapters 30.61 through 30.67 SCC for environmental <br /> standards and mitigation, as applicable. <br /> 2. Where utility installation or other construction has impacted streams, wetlands, <br /> fish and wildlife habitat areas or their buffers, full restoration and/or mitigation <br /> shall be performed as required by franchise, permit or county code. <br /> 3.
