4.1 STATEMENT OF COMPLIANCE <br />In accordance with W.A.C. 296-62-00721 and PSCAA Regulation III, Article 4, Washington Asbestos Testing LLC <br />performed an asbestos survey for property address 3217 Lombard Ave, Everett, WA, 98201. Should employees <br />or contract personnel encounter any suspect ACM (asbestos containing materials) it is their responsibility to: <br />1. Contact a representative of the owner or the owner themselves. <br />2. Consult the inspection report (survey) to determine whether or not the suspect material contains <br />asbestos. <br />3. If the suspect material was not sampled and analyzed in the inspection report (survey), then the <br />material must be presumed to contain asbestos until proven otherwise by sampling the new material <br />and undergoing PLM analysis. <br />4. Make sure to notify all employees and contractors working on this project of the location and type of <br />materials found to contain asbestos. <br />No asbestos was detected in any samples collected and analyzed. <br />The exterior gray trim paint tested positive for lead. <br />The rear deck red paint tested positive for lead. <br />The exterior shed brown paint tested positive for lead. <br />The white paint located in the basment tested positive for lead. <br />I hereby attest: <br />The inspection report (survey) has been made available to me.
