STRUCTURAL STEEL OBSERVATIONS REPORT MAYES I lrerracon <br /> Report Number: M7221502.0015 <br /> Service Date: 11/21/22 20225 Cedar Valley Rd Ste 110 <br /> Report Date: 11/23/22 Lynnwood,WA 98036-6365 <br /> 425-742-9360 <br /> Client Project <br /> The Boeing Company Boeing Everett 45-04 Paint Hangar Wing Stands <br /> Attn: Mark Gosnold 2451 100th St SW <br /> PO BOX 3707 Everett,WA <br /> Seattle, WA 98124-2207 <br /> Project Number: M7221502 <br /> Permit No.:Not Applicable <br /> GENERAL INFORMATION <br /> Mayes Terracon performed inspection of welding on the air damper support steel in Building 45-04 for the <br /> replacement of new ventilation dampers. <br /> Visual inspection was performed on the welding of the HSS upper course members of the new damper supports. <br /> Inspection found welding on these members in compliance with the structural drawings and AWSA D1.1. <br /> CORRECTIVE ACTION TAKEN <br /> On the previous inspection visit on 11/10/22 inspection noted that the last HSS member in the upper and lower <br /> courses on the west end was short and left an excessive gap between the horizontal member. The contractor has <br /> removed this member and installed a new member that is in compliance with the structural drawings. Additionally, <br /> the inspector received a copy of an email from Coffman Engineers, dated 11/11/2022, that would allow welding <br /> on the top and bottom of the new HSS member and increasing the weld size to 1/4". This clears Mayes Terracon <br /> non-conforming condition reported in report M7221502.0011, dated 11/10/22. <br /> Inspection found these welds in compliance with the structural drawings, RFI 100 and AWS D1.1. <br /> COMPLIANCE STATEMENT <br /> To the best of our knowledge, the items inspected today are in conformance with approved plans and <br /> specifications. <br /> The excess gap on the upper course HSS reported on 11/10/22 have been corrected. <br /> Services: Special Inspections/Materials Testing <br /> Mayes/Terracon Rep.: Michael Virgilio <br /> Reported To: Skanska USA Building Inc <br /> Contractor: Skanska <br /> Report Distribution: <br /> See attached distribution list. <br /> Reviewed By: <br /> Barry Tuttle <br /> Project Field Manager <br /> The tests were performed in general accordance with applicable ASTM,AASHTO,or DOT test methods.
