MR#2: Construction Storm water Pollution Prevention <br /> a. Element 13 must be expanded to address the permeable pavement <br /> installation. <br /> iv. MR#9: Operation and Maintenance <br /> a. The maintenance and operation information must be expanded to <br /> include the permeable pavement driveways. <br /> b. The maintenance and operation information for the stormwater <br /> facilities must be provided in a separate document entitled <br /> "Maintenance and Operations Manual". This document must include a <br /> first page indicating the party responsible for maintenance and <br /> operation. <br /> c. The following language shall also be included in both the M&O manual <br /> and the drainage report: <br /> A copy of the maintenance and operations manual shall be retained <br /> on-site or within reasonable access to the site and shall be <br /> transferred with the property to any new owner. A log of <br /> maintenance activities that indicate what actions were taken shall be <br /> kept and be available for inspection by the City of Everett upon <br /> request. <br /> d. The appropriate maintenance checklists from the Stormwater <br /> Management Manual shall be included with the M&O Manual. <br /> v. The following section of the 2014 SWMMWW must be addressed for this site: <br /> Volume I, Section 3.1.7, page 3-16 <br /> Declaration of Covenant for Privately Maintained On-site <br /> Stormwater.Management BMPs <br /> To ensure future maintenance and allow access for inspection by the local <br /> government, any On-site Stonnwater Management BMPs for which the <br /> applicant identifies operation and maintenance to be the responsibility of a <br /> pnvate party must have a declaration of covenant and grant of easement. <br /> Design details, figures, and maintenance instructions for each On-site <br /> Sto mwater Management BMP shall be attached. A map showing the <br /> location of newly planted and retained trees claimed as flow reduction <br /> credits shall also be attached. This applies to every lot within a subdivision <br /> on which an On-site Stormwater Management BMP is proposed. After <br /> approval by the local government,the declaration of covenant and grant of <br /> easement must be signed and recorded at the appropriate records office of <br /> the local government. <br /> Page 2 of 3 <br />
