EXHIBIT A1 <br />City of Everett – Replacement of Reservoir 3 <br />Contract Amendment No 1 2 Scope of Services <br />Task 100 – Project Management <br />Task Objective <br />Monitor, control, and adjust scope (upon mutual agreement), schedule, and budget as well as <br />provide monthly status reporting, accounting, and invoicing and project coordination with the <br />City. <br />Consultant Services <br />1. The amendment increases the fee for additional geotechnical coordination from sub- <br />consultant (HWA GeoSciences) with design team due to potential per- and <br />polyfluoroalkyl substances (PFAS) impacts and change in foundation design due to <br />differing site conditions. <br />Client Responsibilities <br />1. Continuation of original scope of work. <br />Assumptions <br />1. This amendment does not increase the contract duration. <br />Deliverables <br />1. Continuation of original scope of work. <br />Task 200 – Preliminary Design Report <br />Task Objective <br />Develop alternatives for replacement reservoirs, identify a preferred alternative, and prepare the <br />Project Report for the proposed water storage improvements for submittal to the Washington <br />State Department of Health (DOH). <br />Consultant Services <br />1. Revise original Preliminary Design Report (PDR) submitted to DOH on April 24th, 2023 to <br />include the single 8 MG reservoir in the Fire Training Facility in lieu of the two 4 MG <br />reservoirs as part of Phase 1 construction. <br />Client Responsibilities <br />1. Continuation of original scope of work. <br />Assumptions <br />1. An additional Preliminary Design Report Review Workshop will not be required. <br />Deliverables <br />1. Draft and Final Preliminary Design Report (PDF) including comment log (Excel format).
