CHANGE ORDER # <br />Project (Procurement) Number: Small Works Roster Project Title: <br />LSWR: 2024-088 Little League Sign Installation <br />Description of Change: (Attach contractor’s change proposal and cost schedule) (Attach Price or Cost Analysis) <br />BUILDING/LOCATION: 145 Alverson Boulevard, Everett, WA 98201 <br />This change order includes additional installation requirements and extends the contract period by 78 <br />days. Originally, a hard completion date was requested to ensure the scoreboard was available for <br />the start of the Little League season. However, the contractor was not able to receive the materials in <br />time to meet the deadline. The contract is being extended to accommodate the long lead time for <br />materials. <br />Additional costs are needed to compensate the contractor for scoreboard design changes approved <br />by the project manager during the graphics approval layout phase dated on 4/19/24.
