The O&M plan must also address long-term activities to assure ongoing pollutant removal and <br />flow-control capability of the project in accordance with the design manual. <br />G. Upon completion of construction, the RECIPIENT will provide to ECOLOGY: <br />1. A Stormwater Construction Completion Form signed by a professional engineer indicating that the project was completed <br />in accordance with the plans and specifications and major change orders approved by ECOLOGY’s Project Engineer and <br />shown on the Record Drawings. The Stormwater Construction Completion Form can be found on the ECOLOGY website . <br />2. GIS compatible project area in Shapefile, Geodatabase file, or ECOLOGY-Approved Equivalent. The project area should <br />include features for treatment facilities and contributing areas. <br />Task Goal Statement: <br />The RECIPIENT will oversee and manage construction , communicate with ECOLOGY in a timely fashion, and provide <br />ECOLOGY with all requested project documentation. <br />Task Expected Outcome: <br />Project will be constructed on schedule and in accordance with accepted plans. <br />Deliverables <br />Due DateDescriptionNumber <br />3.1 Construction Quality Assurance Plan. Submit to ECOLOGY for review <br />and acceptance. Upload to EAGL and notify ECOLOGY when upload <br />is complete. <br />3.2 Pre-construction conference meeting minutes. Upload to EAGL and <br />notify Ecology when upload is complete. <br />3.3 Project Schedule. Upload to EAGL using naming convention D 3.3 <br />SCHEDULE MO-DA-YEAR and notify Ecology when upload is <br />complete. <br />3.4 Revised Cash Flow Estimates when changes in construction schedule <br />occur. Upload to EAGL using naming convention D 3.4 CASHFLOW <br />MO-DA-YEAR and notify Ecology when upload is complete. <br />3.5 Change Order(s). Upload to EAGL and notify Ecology when upload is <br />complete. <br />3.6 Copy of Facility Operation and Maintenance Plan. Submit to <br />ECOLOGY for review and acceptance .
