CITY OF EVERETT WEST MARINE VIEW DRIVE/ALVERSON AND 41 ST ST/GRAND AVE <br />PEDESTRIAN IMPROVEMENTS <br />STATE FUND# HLP-PB15(032) <br />WO #3630 July 25, 2024 <br />21 <br />1 sum item will be paid upon submittal and approval of the completed Record Drawings set <br />2 prepared in conformance with these Special Provisions. <br />3 <br />4 A minimum bid amount has been entered in the Bid Proposal for this item. The Contractor <br />5 must bid at least that amount. <br />6 <br />7 1-06.GR1 <br />8 Control of Material <br />9 <br />10 1-06.1.GR1 <br />11 Approval of Materials Prior to Use <br />12 <br />13 1-06.1.INST1.GR1 <br />14 Section 1-06.1 is supplemented with the following: <br />15 <br />16 1-06.1.OPT1.GR1 <br />17 (April 3, 2017) <br />18 For each proposed material that is required to be submitted for approval using either the <br />19 QPL or RAM process the Contractor will be allowed to submit for approval two material <br />20 sources or manufacturers per material type at no cost. Additional material sources or <br />21 manufacturers may be submitted for approval and will be processed at a cost of $125.00 <br />22 per material source or manufacturer submitted by QPL submittal and $400.00 per material <br />23 submitted by RAM. All costs for processing additional material sources or manufacturers <br />24 will be deducted from monies due or that may come due to the Contractor. Subject to a <br />25 request by the Contractor and a determination by the Engineer the costs for processing <br />26 may be waived. <br />27 <br />28 1-06.6.RTF <br />29 1-06.6 Recycled Materials <br />30 (January 4, 2016 APWA GSP) <br />31 <br />32 Delete this section, including its subsections, and replace it with the following: <br />33 <br />34 The Contractor shall make their best effort to utilize recycled materials in the construction <br />35 of the project. Approval of such material use shall be as detailed elsewhere in the <br />36 Standard Specifications. <br />37 <br />38 Prior to Physical Completion the Contractor shall report the quantity of recycled materials <br />39 that were utilized in the construction of the project for each of the items listed in Section 9- <br />40 03.21. The report shall include hot mix asphalt, recycled concrete aggregate, recycled <br />41 glass, steel furnace slag and other recycled materials (e.g. utilization of on-site material <br />42 and aggregates from concrete returned to the supplier). The Contractors report shall be <br />43 provided on DOT form 350-075 Recycled Materials Reporting. <br />44 <br />45 1-07.GR1 <br />46 Legal Relations and Responsibilities to the Public <br />47
