<br />Exhibit A (Scope of Work) <br />V. Method of Compensation: <br /> <br />As an independent contractor Bridgeways shall submit monthly invoices and be <br />reimbursed lump sum rates for providing the staff required to meet the program <br />requirements and client needs at the following rates: <br /> <br />Personnel Costs Monthly Invoice Annual Maximum <br />Cost <br />Program Supervision $2,400 $28,800 <br />SUD Professional Assessments and <br />Treatment $6,250 $75,000 <br />Case Management /Liaison Services $7,400 $88,800 <br />Personnel Total (Subcontracted) $16,050 $192,600 <br /> <br />Bridgeways shall provide supporting documentation for all other allowable <br />expenses as is required per each spending category. Any other miscellaneous <br />expenses directly related to program delivery or participant recovery supports <br />require pre-approval of the court. Direct requests from the Presiding Judge both in <br />and out of court sessions will be considered by Bridgeways as an authorization <br />unless otherwise directly instructed. <br /> <br />
