City of Everett 11 Street Medicine Team Pilot Program <br />Subcontract of HCA Contract #K7759 <br /> <br />Address: 2930 Wetmore Ave, Suite 10A <br />Everett, WA 98201 Address: 809 W Main St <br />Ste C <br />Monroe, WA 98272 <br />Phone: 425-257-7120 Phone: 425-387-1863 <br />Email: jwillie@everettwa.gov Email: jacobg@conquerclinics.com <br /> <br />3.6 KEY STAFF <br /> <br />3.6.1 Except in the case of a legally required leave of absence, sickness, death, <br />termination of employment or unpaid leave of absence, Key Staff must not be <br />changed during the term of the SOW from the people who were described in the <br />Response for the first SOW, or those Key Staff initially assigned to subsequent <br />SOWs, without the prior written approval of the City until completion of their <br />assigned tasks. <br />3.6.2 During the term of the SOW, the City reserves the right to approve or disapprove <br />Conquer <br />proposed changes in Conquer <br />reassignment of any Conquer staff found unacceptable by the City, subject to the <br />City Conquer must provide a <br />resume to the City of any replacement Key Staff and all staff proposed by <br />Conquer as replacements for other staff must have comparable or greater skills <br />for performing the activities as performed by the staff being replaced. <br />3.7 LEGAL NOTICES <br /> <br />Any notice or demand or other communication required or permitted to be given under this <br />Contract or applicable law is effective only if it is in writing and signed by the applicable <br />party, properly addressed, and delivered in person, via email, or by a recognized courier <br />service, or deposited with the United States Postal Service as first-class mail, postage <br />prepaid certified mail, return receipt requested, to the parties at the addresses provided in <br />this section. <br />3.7.1 In the case of notice to the City: <br /> <br />Attention: Julie Willie <br />City of Everett <br />2930 Wetmore Ave, Suite 10A <br />Everett, WA 98201 <br />jwillie@everettwa.gov <br />3.7.2 In the case of notice to Conquer:
