City of Everett 4 Street Medicine Team Pilot Program <br />Subcontract of HCA Contract #K7759 <br /> <br />Recitals <br /> <br />The State of Washington, acting by and through the Health Care Authority (HCA), entered into <br />the Master Contract with the City of Everett to secure the creation of a street medicine team <br />pilot program in accordance with the legislative mandate in ESSB 5950; and <br /> <br />The City has determined that entering into this Contract will meet the need to establish such a <br />street medicine program. This Contract is a subcontract to the Master Contract. <br />IN CONSIDERATION of the mutual promises as set forth in this Contract, the parties agree as <br />follows: <br />1. STATEMENT OF WORK (SOW) <br /> <br />Conquer will provide the services and staff as described in Attachment 1: Statement of Work. <br />2. DEFINITIONS <br /> <br /> means a person to whom signature authority has been <br />delegated in writing acting within the limits of his/her authority. <br /> means an individual or individuals with an authorized business need to <br />or Confidential Information under this Contract. <br />means a Business Associate as defined in 45 C.F.R. § 160.103, who <br />performs or assists in the performance of an activity for or on behalf of HCA or the City, that <br />involves the use or Disclosure of Protected Health Information (PHI). Any reference to <br />officers, Conquer Subcontractors, third party contractors, volunteers, or directors. <br /> Associate or means an agreement that includes the <br />Business Associate provisions required by the U.S. Department of Health and Human <br />Services, Office for Civil Rights. <br /> means Monday through Friday, 8:00 a.m. to 5:00 p.m., Pacific Time, except <br />for holidays observed by the state of Washington.
