(Krazan) appreciates the opportunity to submit this Proposal/Cost Estimate to Everett Parks and <br />Recreation Department for a limited hazardous materials (hazmat) survey at the above-mentioned project site. <br />SITE AND PROJECT DESCRIPTION <br />Everett Parks and Recreation Department has requested this proposal for a limited hazmatsurvey on the referenced property <br />which consisted of a gazebo and platform totaling approximately 25 feet x 25 feet located within the Clark Park in Everett, <br />Washington. Krazan understands that Everett Parks and Recreation Department has requested this survey as a matter of <br />due diligence prior to demolition activities. Current federal, state, and local regulations require that property owners, prior <br />to renovation and/or demolition work on existing properties, conduct surveys to locate and identify hazardous or potentially <br />hazardous materials. The scope of this limited hazmat survey has been client-defined and is intended to provide additional <br />information to Everett Parks and Recreation Department as an integral part of their due diligence process. The findings of <br />such surveys are required to be made available to contractors and subcontractors providing services should renovation or <br />demolition be undertaken at the site. Additional codes, standards, and regulations dictate the handling of those hazardous <br />materials identified during the survey. The proper identification and assessment of such materials also facilitate the <br />development of plans and technical specifications that minimize the potential liabilities of owners and contractors. <br />LIMITED HAZMAT SURVEY <br />Krazan proposes to conduct a limited hazmat survey to identify suspect asbestos-containing materials (ACM) and lead- <br />based paints (LBP), which may be impacted during any future renovations and/or demolition. To comply with the current <br />requirements of federal, state, and local regulatory agencies, Krazan anticipates that the Hazmat survey will involve <br />destructive sampling techniques and selective demolition to access hidden or concealed suspect ACM/LBP. However, it <br />is understood that the sampling will be limited in scope based on the approval of the property owner, and areas defined by <br />the client. The property owner will verify and authorize selective areas for destructive sampling. If areas of suspect <br />ACM/LBP are unattainable for sampling, those areas will be included in the report as presumed ACM/LBP and will require <br />sampling before any renovations and/or demolition. Repair of selective demolition areas is beyond the scope of these <br />services. <br />Krazan proposes to perform this work on a time & expense basis. Krazan will collect and submit samples of all suspect <br />ACM/LBP encountered at the site to an accredited and approved laboratory. Upon receipt of laboratory results, Krazan <br />will prepare a written report summarizing findings, conclusions, and recommendations. Krazan will only include areas of <br />the structure that have beenapproved by the owner of the property and defined by the client. This survey will be conducted <br />by an Asbestos Hazard Emergency Response Act (AHERA) Building Inspector.
